2007 Update to guidelines for early management of stroke

In this recent update of the 2003 guidelines, a writing group sponsored by the Stroke Council of the American Heart Association, reviewed all the evidence supporting specific practices aimed at the management of the acute ischemic stroke patients. There are no great deviations from the previous guidelines in terms of pharmacotherapy, but more emphasis is placed on pre-hospital systems of care and stroke center designations. No new evidence exists to change recommendations on thrombolytic therapy, antiplatelet or anticoagulant therapy but future directions in neuroprotection and combination reperfusion therapy are explored.
These guidelines are very helpful to the clinician interested in standardizing care for ischemic stroke patients and they serve as a starting point for quality improvement initiatives.
